1. Executive Summary

The Cardiology Intelligence Agent is a production-grade, RAG-powered clinical decision support system built for the HCLS AI Factory precision medicine platform running on NVIDIA DGX Spark. It delivers evidence-based cardiovascular guidance across the full spectrum of cardiology subspecialties -- from heart failure management and coronary artery disease to electrophysiology, valvular disease, cardiac imaging, preventive cardiology, and cardio-oncology -- by combining 12 domain-specific Milvus vector collections, six validated risk calculators, a four-pillar GDMT optimization engine, 18 cross-modal genomic triggers, and an autonomous reasoning pipeline that plans, searches, evaluates, and synthesizes clinical evidence in real time.

The agent is architected as a three-tier system: a Streamlit UI (port 8536) for interactive clinical exploration, a FastAPI REST API (port 8126) exposing 33 endpoints for programmatic integration, and a RAG engine backed by Milvus (port 19530) with BGE-small-en-v1.5 384-dimensional embeddings. All 11 clinical workflows, all 6 risk calculators, the GDMT optimizer, and the full query expansion system operate independently of Milvus connectivity, ensuring graceful degradation and robust demo capability even when the vector store is unavailable.

The codebase comprises 58 Python files totaling 44,451 lines of code, with 20 dedicated test files containing 11,450 lines and 1,966 passing tests at a 100% pass rate (1.08s execution time). The knowledge graph contains 45 cardiovascular conditions, 56 genes, 32 drug classes, 29 cardiac biomarkers, 15 imaging modalities, and 51 guideline recommendations from ACC/AHA/ESC sources. The Streamlit UI communicates with the FastAPI backend via HTTP REST calls. The API layer dispatches queries through the RAG engine for evidence retrieval, the risk calculator engine for validated scoring, the GDMT optimizer for heart failure therapy recommendations, and the clinical workflow engine for condition-specific assessments. The RAG engine performs parallel multi-collection vector search against Milvus using workflow-specific collection weights, applies query expansion via 18 synonym maps, scores citations, and synthesizes responses through the Claude LLM. Cross-modal genomic triggers fire when clinical findings meet predefined thresholds (e.g., unexplained LVH >= 15mm), bridging bedside cardiology with the HCLS AI Factory's genomic pipeline on the same DGX Spark.
